Attention Rosebud Sioux Tribe Community,
It's 2021 Head Start/Early Head Start Round Up time! This is a perfect opportunity for all parents to complete your child’s enrollment packet information and obtain your child’s physical and dental screening for the 2021-2022 school year. Parents please bring any missing information required to get child registered for school. Early Head Start families will need to update their physicals and dental as well.
Remember, the 2021 Round Up will be in the classrooms this year, so please come for the day and get your child enrolled!
We will be about in the following Head Start/Early Head Start classrooms on these dates;
July 1st @ Winner Head Start Center
July 2nd @ White River Head Start Center
July 6th @ St. Francis Head Start Center
July 7th @ Norris Head Start Center
July 8th @ Parmelee Head Start Center
July 9th and 12th @ Mission Head Start Center
July 13th and 14th @ Rosebud Head Start Center
If you do show up, you will need the following forms to complete your child's enrollment process;
• Completed Enrollment packet
• Dental Screening
• Physical Examination (Including Screening/Hemoglobin Screening) - Provided ON HEAD START CENTERS!
• Medicaid Card
• Child's Birth Certificate
• Abstract of Pending Letter
• Recent Income Verification
• IEP (If Available)
• Updated Immunization
There will be drawings and door prizes, PHYSICALS AND DENTAL SCREENINGS WILL BE COMPLETED AT EACH LOCATION. Breakfast and Lunch will be provided to all!
ALL Head Start/Early Head Start Round Up days are scheduled from 9:00am to 3:00 daily at the Head Start Centers. We cannot go further than 3:00pm so PLEASE be timely!
